29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: a) On or about July 14, 2025 at 1579 SE North Blackwell Dr, Port Saint Lucie, FL 34952, employees installing drippage on a residential roof were exposed to a fall hazard of approximately 8-feet 4-inches while accessing an upper landing using an unsecured ladder that did not extend at least 3 feet above the landing surface.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) On or about July 14, 2025 at 1579 SE North Blackwell Dr, Port Saint Lucie, FL 34952, employees were exposed to an approximate 8-foot 4-inch fall hazard while installing drippage on a residential roof without a means of fall protection. Code Red Roofers, Inc. was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard 29 CFR 1926.501(b)(10), which was contained in OSHA inspection number 1719725, citation number 1, item number 1, and was affirmed as a final order on July 12, 2024, with respect to a workplace located at 5405 Turnpike Feeder Road, Fort Pierce, Florida 34951.
