29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a)Dakota Foundry, Inc. dba Dakota Foundry at 20 Park Lane, Webster, SD 57274: On or about and times prior to July 15, 2025, the employer did not ensure employees were protected from caught-in hazards between the Kloster mold rollover unit and the frame rollers during rotation. This condition exposed employees to caught-between hazards. (b)Dakota Foundry, Inc. dba Dakota Foundry at 20 Park Lane, Webster, SD 57274: On or about and times prior to July 15, 2025, the employer did not ensure employees were protected from caught-in hazards between the mold cart rollers and the track during operation of the molding line. This condition exposed employees to caught-between hazards. (c)Dakota Foundry, Inc. dba Dakota Foundry at 20 Park Lane, Webster, SD 57274: On or about and times prior to July 15, 2025, the employer did not ensure employees were protected from caught-in hazards created between the mold clamp and the mold during operation of the Kloster mold rollover unit. This condition exposed employees to caught-in hazards. (d)Dakota Foundry, Inc. dba Dakota Foundry at 20 Park Lane, Webster, SD 57274: On or about and times prior to July 15, 2025, the employer did not ensure employees were protected from caught hazard created between the fins of the conveyor rollers and the nearby structure of the main floor hot shaker table during operation.
