Tallmadge Ave, Tallmadge Ohio 44278; during the installation of roofing material on a commercial roof, the employer did not ensure portable ladders extend 3 feet above the upper landing surface.

29 CFR 1926.501(b)(10):Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system: On or about July 10, 2023, during a roof replacement of a commercial building located at 1155 E. Tallmadge AVE, Tallmadge Ohio 44278, the employer did not ensure employees working from a low-sloped roof were protected from a fall of approximately 11.7 feet by the use of a guardrail system, safety net system, personal fall arrest system and/or combination warning line system and safety monitoring system. J E Palma Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.501(b)(10) Duty to have fall protection), which was contained in OSHA inspection number 1684075, citation number 01, item number 01 and was affirmed as a final order on 8/23/2023, with respect to a workplace located at 6385 Cochran Rd, Solon, OH 44139. J E Palma Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.501(b)(10) Duty to have fall protection), which was contained in OSHA inspection number 1624344, citation number 01, item number 002 and was affirmed as a final order on 04/10/2023, with respect to a workplace located at 100 North Walnut Street, Gnadenhutten, OH 44629.
