29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames, and uprights did not bear on base plates and mud sills or other adequate firm foundation:  (a) 4807 Long beach Blvd, Beach Haven, NJ 08008: The employee was working from a pump jack scaffold where the middle pole was not resting on a base plate and mud sill. The pump jack scaffold was very unstable when the employee was walking on it. Condition observed on or about 07/17/2025.  The Direct Builders Trade was previously cited for a violation of this occupational safety and health standard or its equivalent standard (name previously cited 1926.451(c)(2), which was contained in OSHA inspection number 1807933, citation number 1, item number 1 and was affirmed as a final order on 05/13/2025, with respect to a workplace located at 11 Charlette Court, Marlton, NJ 08053.  NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.  (a) 4807 Long beach Blvd, Beach Haven, NJ 08008: The employee was engaged in siding installation and other related activities was exposed to fall hazards, roughly 20 feet to the ground surface below. The employee was working from a pump jack scaffold without the use of a personal fall arrest system or guardrails.   The Direct Builders Trade was previously cited for a violation of this occupational safety and health standard or its equivalent standard (name previously cited 1926.451(g)(1)(vii), which was contained in OSHA inspection number 1807933, citation number 1, item number 2 and was affirmed as a final order on 05/13/2025, with respect to a workplace located at 11 Charlette Court, Marlton, NJ 08053. (a) 4807 Long beach Blvd, Beach Haven, NJ 08008: The employee was exposed to a fall hazard when working from a pump jack scaffold. The pump jack scaffold was not being braced properly and was unstable when the employee was walking and working on it. Condition observed on or about 07/17/2025. 29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a) 4807 Long beach Blvd, Beach Haven, NJ 08008: Worker was exposed to fall hazards of greater then 20 feet to the ground surface below. The worker was accessing and egressing from a deck using a portable ladder whose side rails did not extend at least 3 feet above the surface being accessed/egressed. Condition observed on or about 07/17/2025.
