Section 5(a)(1) of the Occupational Safety and Health Act: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees, in that employees were exposed to amputation and laceration hazards.   a) On or about July 17, 2025, at 1926 Skees Road, West Palm Beach, FL 33411, employees were exposed to laceration and amputation hazards when accessing the control panel and working near Plant 1 mold press feed conveyor belt.   b) On or about July 17, 2025, at 1926 Skees Road, West Palm Beach, FL 33411, employees were exposed to laceration and amputation hazards when accessing the control panel and working near Plant 2 mold press mud feed conveyor.    c) On or about July 17, 2025, at 1926 Skees Road, West Palm Beach, FL 33411, employees were exposed to laceration and amputation hazards when accessing the control panel and working near the block rack chain conveyor.    d) On or about July 17, 2025, at 1926 Skees Road, West Palm Beach, FL 33411, employees were exposed to laceration and amputation hazards when accessing the control panel and working near the green/raw block conveyor.   e) On or about July 17, 2025, at 1926 Skees Road, West Palm Beach, FL 33411, employees were exposed to laceration and amputation hazards when accessing the control panel and working near the dry block conveyor.     f) On or about July 17, 2025, at 1926 Skees Road, West Palm Beach, FL 33411, at outdoor South side aggregate conveyor between Plant 1 and Plant 2, employees were exposed to laceration and amputation hazards when accessing the control panel and working near conveyor.     g) On or about July 17, 2025, at 1926 Skees Road, West Palm Beach, FL 33411, at Plant 3, employees were exposed to laceration and amputation hazards when accessing the control panel and working near the outdoor North side material conveyor.
