29 CFR  1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE).  On or about July 21, 2025, and times prior thereto, near the northeastern corner of the backyard, employees work next to energized electrical cables rated at 7,620-volts and the assessment was not performed to determine if there were hazards present.

29 CFR 1910.138(a): The employer did not select and require employee(s) to use appropriate hand protection when employees' hands were exposed to hazards such as those from skin absorption of harmful substances; severe cuts or lacerations; severe abrasion; punctures; chemical burns; thermal burns; and harmful temperature extremes.  On or about July 21, 2025, and times prior thereto, near the northeastern corner of the backyard, an employee was not in use of appropriate insulated gloves while working on and next to energized electrical cables rated from 240-volts to 7,620-volts.

29 CFR 1910.269(l)(2)(i)(A): Except as provided in paragraph (l)(2)(ii) of this section, at least two employees were not present while any employees perform work such as installation, removal, or repair of lines energized at more than 600 volts.   On or about July 21, 2025, and times prior thereto, near the northeastern corner of the backyard, the employer did not ensure at least two employees were present while an employee was installing a temporary 240-volt cable in a pad-mounted transformer with an uninsulated energized 7,620-volt electrical cable.

29 CFR 1910.269(l)(4)(i): The employer did not ensure that when an employee uses rubber insulating gloves as insulation from energized parts (under paragraph (l)(3)(iii)(A) of this section), the employee also uses rubber insulating sleeves.   On or about July 21, 2025, and times prior thereto, near the northeastern corner of the backyard, an employee working on and next to energized electrical cables was not in use of rubber insulating sleeves while in use of rubber insulating gloves.
