29 CFR  1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  a) On or about 07/24/2025, in the tool room; Employees were exposed to caught in hazards when Lockout/Tagout procedures were not utilized for the purpose of controlling hazardous energy when performing servicing and maintenance activities on machines such as, but not limited to changing wheels on the Okamoto 12-24 DX V surface grinder.  b) On or about 07/24/2025, in the New addition Davenport area; Employees were exposed to crushed by hazards when Lockout/Tagout procedures were not utilized for the purpose of controlling hazardous energy when performing servicing and maintenance activities on machines such as, but not limited to cleaning the Davenport #147, #148 and #149 machines.  c) On or about 07/24/2025, In the Automotive area; Employees were exposed to crushed by hazards when Lockout/Tagout procedures were not utilized for the purpose of controlling hazardous energy when performing servicing and maintenance activities on machines such as, but not limited to replacing bar stock on the Davenport #126, #123, and #128 machines.  Instance a) penalty is amnded to $4000.00.  Instances b) and c) "withdrawn" and the enhanced abatement required to make sure employees cannot reach the machines' hazardous areas are: Instance b) Employer will develop and implement a stardard operating procedure (SOP) for cleanign chips, train exposed employees and do periodic audits to ensure compliance. Instance c) Employer will provide machine specific training to employees related to machine functions (i.e. "jog" button) to prevent injuries. ABATEMENT DOCUMENTATION REQUIRED.
