29 CFR  1926.405(g)(2)(iv):Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:  At a worksite located at 118 W Walnut Street, Raymore, MO, employees were exposed to electrical hazards that include but are not limited to shock, electrocution and electrical fire hazards.  The employer exposed the employees to these hazards by allowing them to utilize an electrical extension cord with strain relief damage to power a circular saw at the worksite.

29 CFR  1926.451(b)(1)(i):Each platform unit (e.g., scaffold plank, fabricated plank, fabricated deck, or fabricated platform) was not installed so that the space between adjacent units and the space between the platform and the uprights were no more than 1 inch (2.5 cm) wide, except where the employer can demonstrate that a wider space is necessary.  At a worksite located at 118 W Walnut Street, Raymore, MO, employees were exposed to fall hazards.  The employer exposed the employees to these hazards by allowing them to work on platforms of a scaffold that had space more than 1 inch wide between adjacent units and between platforms and uprights.  Instances of exposure include, but are not limited to:  Instance 1:  Two employees working from a third tier platform supported by outrigger extension brackets, that faced an eastern section of the structure.  Instance 2:  One employee working from a second tier platform of the scaffold, that faced an eastern section of the structure.    Instance 3:  One employee working from a third tier platform of the scaffold that faced a northern section of the structure.  Instance 4:  One employee working from a first tier platform of the scaffold that faced a northern section of the structure.

29 CFR  1926.451(e)(1):When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  At a worksite located at 118 W Walnut Street, Raymore, MO, employees were exposed to fall hazards.  The employer exposed the employees to these hazards by allowing them to access platforms of a scaffold without providing an approved means of access to the platforms.   Instances of exposure include, but are not limited to:  Instance 1:  Two employees accessing a third tier platform supported by outrigger extension brackets, that faced an eastern section of the structure.  Instance 2:  One employee accessing a second tier platform of the scaffold, that faced an eastern section of the structure.    Instance 3:  One employee accessing a third tier platform of the scaffold that faced a northern section of the structure.
