1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person: a) On or about July 21, 2025, Gates Enterprises, as the controlling contractor did not ensure a safety program was developed and implemented for their work environment, to include regular inspections made by a competent person of the job-site, materials, and equipment. Subcontractor employees performing roofing work without utilizing a fall protection system. This condition exposed subcontractor employees to a fall hazard of greater than 6 feet.
