29 CFR  1926.451(c)(2)(i): Footings were not level, sound, rigid, and capable of supporting the loaded scaffold without settling or displacement:  a) 950 Brentwood Street Lakewood, CO 80214: On or about July 28, 2025, and at times prior, employees installing siding were exposed to unsafe scaffolding hazards, such as a potential falls, while working on a pump jack scaffold. The employer failed to ensure the footings were level and would not pose a risk for settling or displacement in that the scaffold was unstable where one of the poles was sitting half way on the wooden base plate and halfway off on uneven ground.  Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR  1926.451(f)(7): Scaffolds were not erected, moved, dismantled, or altered, by trained and experienced employees under the supervision and direction of a competent person qualified in scaffold erection, moving, dismantling or alteration:; Such activities were not performed only by experienced and trained employees selected for such work by the competent person.  a) 950 Brentwood Street Lakewood, CO 80214: On or about July 28, 2025, and at times prior, one employee installing siding was exposed to unsafe scaffolding hazards, such as a potential fall, while working on a pump jack scaffold. The employer failed to ensure a competent person constructed the scaffold. The manager on site did not provide brackets for the employees and instructed them to install incorrect guardrails systems. The scaffold had open sides and ends of the platform. The two edges to enter or exit were open and the employee using the scaffold crafted 2x4s around the masts, in order to nail on 2x4 mid and top rails. In addition, the scaffold was unstable where one of the masts or poles was sitting half way on the wooden base plate and halfway off, ready to fall into the center of the plate's hole.  Abatement certification and documentation are required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR  1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms.  a.) 980 Brentwood Street Lakewood, CO 80214: On or about July 28, 2025, and at times prior, one employee installing siding was exposed to fall hazards greater than 10 feet while working on a pump jack scaffold. The employer failed to ensure employees were protected from falls by installing the appropriate guardrails systems (e.g. brackets that come from the manufacturer) along all open sides and ends of the platform. The two edges to enter or exit were open and the employee using the scaffold crafted 2x4s around the masts, in order to nail on 2x4 mid and top rails.  Abatement certification and documentation are required for this item (see enclosed "Certification of Corrective Action Worksheet").

The employer failed to ensure employees were protected from falls by utilizing a fall protection system or guardrails.
