29 CFR 1910.242(a):Hand and portable powered tools or equipment were not kept in safe condition. At the jobsite located at 4530 E 75th Terrace, Kansas City, MO 64132, Employees were expose to struck by hazards while operating a cut-off saw with a modified cut-off wheel. Employees were required to utilize a machinists file to enlarge the cutoff wheel's 1/4" arbor hole to fit the cutoff saw's 3/8" arbor.

29 CFR 1910.243(c)(1):Abrasive wheel(s) were used on portable grinder(s) which were not provided with safety guard(s) meeting the requirements specified in 29 CFR 1910.243(c)(1) through (c)(4)(a). At the jobsite at 4530 E. 75th Terrace, Kansas City, MO 64132, employees were exposed to struck-by hazards while operating an unguarded cutoff saw. Employees used the cutoff saw to remove pieces of metal during welding and fabrication activities. Employees also used a die grinder fitted with a cutoff wheel. The die grinder was not equipped with a guard, and the model does not have the capability to accept a guard.
