29 CFR 1910.134(e)(1): The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace: On or about July 30, 2025, employees engaged in welding and grinding operations voluntarily wore a Miller T94i-R Powered Air Purifying Respirator (PAPR) with P100 particulate cartridge and the employer did not provide each employee with a respiratory medical exam and medical clearance prior to use.
