OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees:  On or about July 30, 2025, employees were exposed to crushing injuries from the operation of two separate 5-ton underhung cranes, and the employer did not conduct frequent and periodic inspections in accordance with ASME B30.11-2004.   These items include but are not limited to the following:  a.	Hoist, b.	Upper limit device on the hoist, c.	Operating mechanisms for proper operation, proper adjustment, and unusual sounds, and d.	Tanks, valves, pumps, lines, and other parts of air or hydraulic systems for leakage.

29 CFR 1910.28(b)(1)(i): Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i):  On or about July 30, 2025, an employee was exposed to fall hazards without fall protection from two separate mezzanines in the company.  These fall hazards instances included the following:  a.	South Building Mezzanine - 8 feet  b.	North Building Mezzanine - 10 feet 2 inches

29 CFR 1910.28(b)(11)(ii): The employer did not ensure that each flight of stairs having at least 3 treads and at least 4 risers is equipped with stair rail systems and handrails as described in 29 CFR 1910.28 Table D-2-Stairway Handrail Requirements:   On or about July 30, 2025, an employee was exposed to fall hazards of up to 8 feet, while accessing the mezzanine in the South Building, due the lack of a stair rail system on the stairway.

29 CFR 1910.147(d)(1): The authorized employee did not have knowledge of the type and magnitude of the energy, the hazards of the energy to be controlled, and the method or means to control the energy before the authorized or affected employee turned off equipment.  On or about July 30, 2025, the employer did not determine through reviews conducted between an observer and authorized employees if employees who performed servicing and maintenance activities on equipment within the facility had sufficient knowledge of the hazards of the energy to be controlled and means of controlling them.
