29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated, the person certifying that the evaluation has been performed, the date(s) of the hazard assessment, and, which identifies the document as a certification of hazard assessment:  On or about July 30, 2025, the employer failed to verify through written certification that a workplace hazard assessment had been performed for employees working in the production area performing tasks to include but not limited to, cleaning and sanitizing of equipment and floors.  PPE such gloves and eye protection are available, but requirements are not documented.

29 CFR 1910.303(b)(6): Conductors or equipment were located in damp or wet locations; where exposed to gases, fumes, vapors, liquids, or other agents that had a deteriorating effect on the conductors or equipment; or where exposed to excessive temperatures and were not identified for use in the operating environment:  On or about July 30, 2025, in the staging area of the facility, the employer allowed electrical equipment such as but not limited to electrical outlets and electrical cords to be used in damp and wet locations. Chemicals including but not limited to AC-55-5-Red are used in the sanitation process in that area and employees are exposed to shock or burn hazards in that equipment is plugged directly into the electrical outlets with the barrier covers removed.

29 CFR 1910.305(e)(1): Cabinets, cutout boxes, fittings, boxes, and panelboard enclosures in damp or wet locations were not installed to prevent moisture or water from entering and accumulating within the enclosures:  On or about July 30, 2025, the employer did not ensure the metal housing, fittings, and conduit of the 240 volt  3 phase electrical push button and lever switch within the staging area of the facility were installed as to prevent moisture or water from entering and accumulating within the enclosures exposing employees to potential shock or burn hazards.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:  On or about July 30, 2025, employees were exposed to potential injury in that the water at the eyewash station located in the chemical room was not at a controlled, low velocity flow or 0.4 gallons per minute that completely rinses eyes and face and is not injurious to the user.
