In the maintenance area, located at 294 SW Harvey Greene Drive, Madison, FL: On or about July 30, 2025, the employer exposed employees to a struck-by hazard, in that procedures were not utilized by employees to control hazardous energy while engaged in mast chain removal for a Mitsubishi Forklift truck during maintenance operations.
