29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii) a) Jobsite located at or near 1618 S 19th Street, Philadelphia PA- Exterior Scaffold- On or about July 31, 2025, the employer did not ensure that each working level of a scaffold was fully planked or decked between the front uprights and guardrail supports. Employees conducting masonry activities were utilizing a scaffold platform that was not fully planked, thereby exposing employees to fall hazards or approximately 19 feet.

29 CFR 1926.451(e)(1): When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be used. Crossbraces shall not be used as a means of access. a) Jobsite located at or near 1618 S 19th Street, Philadelphia PA- Exterior Scaffold- On or about July 31, 2025, the employer did not ensure employees were provided safe means to access and egress the scaffold. Employees utilized the scaffold supports to access the 3rd level of the scaffold platform, thereby exposing employees to fall hazards of approximately 19 feet. New MB General Contractors, LLC was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1538223, Citation Number 1, Item Number 3 and was affirmed as a final order on 1/19/2022, with respect to a workplace located at 1754 N 22nd St, Philadelphia, PA.
