Condition was observed on or about November 26, 2025.

29 CFR  1926.503(a)(1):The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a) Jobsite located at 3401 Hamilton St. Philadelphia, PA, 19104 - On or about November 25, 2025, the employer did not provide effective training for an employee conducting masonry activities on a concrete masonry unit wall without fall protection approximately 10 feet above the ground level, in the recognition and avoidance of fall hazards, thereby exposing employees to fall hazards.  b) Jobsite located at 3401 Hamilton St. Philadelphia, PA, 19104 - On or about November 26, 2025, the employer did not provide effective training for an employee conducting masonry activities from a scaffold without fall protection approximately 14 feet above the ground level, in the recognition and avoidance of fall hazards, thereby exposing employees to fall hazards.

Condition was observed on or about November 26, 2025.  New MB General Contractors LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(e)(1), which was contained in OSHA inspection number 1538223, citation number 1, item number 3 and was affirmed as a final order on 1/19/2022, with respect to a workplace located at 1754 N 22nd St, Philadelphia.

Condition was observed on or about November 26, 2026   New MB General Contractors LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.451(g)(1)(vii), which was contained in OSHA inspection number 1538223, citation number 1, item number 4 and was affirmed as a final order on 1/19/2022, with respect to a workplace located at 1754 N 22nd St, Philadelphia, PA.
