29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about July 31, 2025, within the shop area of the facility, employee(s) engaged in machining operations worked without adequate machine guarding to protect against hazards created by point of operation, ingoing nip points, rotating parts, flying chips and/or sparks. a) Machine #1: Employee(s) utilized a Kingston lathe machine (Machine #24) for machining purposes without guard(s) to protect against point of operation injuries and contact with the rotating chuck. b) Machine #2: Employee(s) utilized a Kingston lathe machine (Machine #25) for machining purposes without guard(s) to protect against point of operation injuries and contact with the rotating chuck. c) Machine #3: Employee(s) utilized a HEM Saw H90-A machine (Machine #26) for machining purposes without guard(s) to protect against point of operation injuries with the moving blade. d) Machine #4: Employee(s) utilized a TechniDrill Deep Hole Drilling Machine for machining purposes without guard(s) to protect against the rotating spindle. Machine Tech, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.212(a)(1), which was contained in OSHA inspection number 1539516, citation number 1, item number 1 and was affirmed as a final order on November 18, 2021, with respect to a workplace located at 203 S.
