29 CFR 1910.242(b): Compressed air, which was not reduced to less than 30 PSI, was used for cleaning purposes without effective chip guarding and personal protective equipment: (a) On or about August 4, 2025, employees using pneumatic airlines when cleaning off equipment and themselves were exposed to hazards created from high pressured air. The employer failed to ensure that the airline(s) which was set at 100 PSI, was equipped with a safety nozzle designed to reduce pressure. Employees, and other workers in the immediate area, were potentially exposed to struck-by hazards from flying debris, hearing damage from loud noise and/or air embolisms.

29 CFR 1910.243(b)(2): Airhose and hose connections used for conducting compressed air to utilization equipment was not designed for the pressure and service to which they were subjected. (a) On or about August 4, 2025, the pneumatic airline used by employees when cleaning off equipment was defective in that the tubing/sheathing connected to the coupling had a hole in it. Pressurized at a 100 PSI, and not equipped with a safety nozzle, employees and other workers in the immediate area were potentially exposed to struck-by hazards from flying debris, hearing damage from loud noise and/or air embolisms.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: On or about August 4, 2025, the following flexible cords/power strips were not used in accordance with manufacturer's instructions, and were used in lieu of permanent wiring: a) Flexible cords used as a power source for the company's Crown electric pallet jack (Model No. ST3000-25, Serial No. 5A536894) located in the manufacturing room, were daisy-chained and had the potential to short circuit leading to electrical overload and fire and/or electric shock/ electrocution if the ground wire becomes to hot. b) Extension cords and relocatable power taps located in the production area were daisy-chained and/or combined in a series to provide power to equipment such as but not limited to computers, iPads, two-way radios, and chargers, leading to electrical overload and fire, if the ground wire becomes to hot.
