29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b): a) 551 Knollwood Blvd Felton, DE. 19943: On or about August 4, 2025, employees were observed performing concrete wall installation activities on a residential construction project while walking on top of precast wall panels approximately 8 feet above the next lower level. The employees were not protected by a guardrail system, safety net system, or personal fall arrest system, nor were any alternative fall protection measures (such as a controlled access zone or scaffolding) provided in accordance with the requirements of 1926.501(b). The employer failed to ensure that fall protection was in place for employees exposed to falls of 6 feet or more, thereby exposing them to the hazard of serious injury or death due to a fall from height.

29 CFR 1926.1053(b)(1): When portable ladders were used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet above the upper landing surface to which the ladder was used to gain access: a) 551 Knollwood Blvd Felton, DE. 19943: On or about August 4, 2025, employees used ladders that did not extend at least 3 feet (0.9 meters) above the upper landing surface as required by the standard exposing employees to fall hazards while transitioning between the ladder and the landing surface, increasing the risk of serious injury.
