29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms. a) On or about August 5, 2025, at the jobsite: subcontractors were exposed to a 13 foot fall hazard while installing block wall from a tubular scaffold in that, the scaffolding was missing guardrails.
