29 CFR 1910.119(e)(1):The process hazard analysis did not identify, evaluate, and address the control of the hazards involved in the process: a) Pyrotechnic Composition Priming Mixer, Exterior Pellet Drying Building: On or about July 30, 2025 and at times prior, employees were exposed to explosion and subsequent fire hazards while cleaning the residual pyrotechnic priming composition from the Husky concrete mixer drum, Serial: 33934. The employer failed to utilize a methodology appropriate to determine and evaluate the hazards of cleaning the mixer after completing the priming process.

The employer failed to ensure that each container of hazardous chemicals in the workplace was labeled with the product identifier and words, pictures, symbols, or combination thereof, which provides at least general information regarding the hazards of the chemicals.
