(a) Job site, Autumn Wood Drive and Patterson Street Extension, Evans City, PA. On or about August 6, 2025, employees were not protected from cave-ins while working in a 16-foot deep excavation in order to install an a 8-inch sewer line system for a new residential development.

29 CFR 1926.651(j)(2):Protection was not provided by placing and keeping excavated or other materials or equipment at least 2 feet (.61m) from the edge of excavations, or by the use of retaining devices that were sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary. (a) Job site, Autumn Wood Drive and Patterson Street Extension, Evans City, PA. On or about August 6, 2025, employees were not protected from falling debris while working in the trench in that there was no setback distance for the spoil pile above the trench exposing employees to struck by hazards while working in the trench.
