29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: (a) Job site, 4680 Sardis Road, New Kensington, PA. On or about February 13, 2026, employees working in an approximately 11-foot deep excavation in order dig a foundation for a residential home, did not have a means of egress in order to exit the excavation. While working in the excavation to mark out the foundation an employee suffered multiple fractures to right leg.

29 CFR 1926.651(j)(1): Adequate protection was not provided to protect employees from loose rock or soil that could pose a hazard by falling or rolling from an excavation face: (a) Job site, 4680 Sardis Road, New Kensington, PA. On or about February 13, 2026, employees working in an approximately 11-foot deep excavation in order dig a foundation for a residential home were not protected from falling debris. While working in the excavation to mark out the foundation an employee suffered multiple fractures to right leg due to falling chunks of soil.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). (a) Job site, 4680 Sardis Road, New Kensington, PA. On or about February 13, 2026, employees were not protected from cave-ins while working in an approximately 12-foot deep excavation in order dig a foundation for a residential home. Employee suffered multiple fractures to right leg while working in the excavation to lay out foundation. The CRS Contracting LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard (name previously cited standard), which was contained in OSHA inspection number 1842270, citation number 1, item number 1-a and was affirmed as a final order on November 20, 2025 with respect to a workplace located at Pattison Street Ext and Autumn Woods Drive in Butler, PA.
