29 CFR 1926.651(h)(1): Employees were working in excavations in which there was accumulated water, or excavations in which water was accumulating, and adequate precautions had not been taken to protect employees against the hazards posed by water accumulation a) Trench: On or about July 31, 2025, and at times prior, the employer exposed employees to cave-in/entrapment hazards, in that the employer did not provide a water removal system to control water accumulation while employees worked in a trench which measured 25 feet long by 10 feet wide by 14 feet deep for the purpose of installing new water line. Employee was outside of the trench box when the collapse occurred.
