Worksite: On or about August 7, 2025, the employer did not ensure powered industrial trucks were operated at a speed that will permit them to be brought to a stop in a safe manner thereby exposing employees to struck-by/crushed-by hazards. The Pet Food Experts, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.178(n)(8), which was contained in OSHA inspection number 1552984, citation number 1, item number 1 and was affirmed as a final order on 12/13/2021, with respect to a workplace located at 1 John Dean Blvd. in Cumberland, RI.
