29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about 6/26/2025, the employer did not protect employees who were engaged in residential roofing operations 6ft (1.8m) or more above lower levels while at the job site located at 5346 Burgundy Ct. in Imperial, MO. The employees were conducting residential roofing operations that exposed them to the hazard of falling from a height of greater than ten (10) feet.   The Jonathan Ayala was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR  1926.501(b)(13), which was contained in OSHA inspection number 1707416, citation number 1, item number 2 and was affirmed as a final order on 3/18/2024, with respect to a workplace located at 214 Stephanie Lynn Ct. in Wentzville, MO.  The Jonathan Ayala was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR  1926.501(b)(13), which was contained in OSHA inspection number 1704177, citation number 1, item number 1 and was affirmed as a final order on 1/3/2025, with respect to a workplace located at 1233 Grand Canyon Drive in Wentzville, MO.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   On or about 6/26/2025, the employer did not ensure portable ladders were constructed and placed so three (3) feet or more were extended above the upper landing at the job site located at 5346 Burgundy Ct. in Imperial, MO. The ladder the employees used to access the roof was extended approximately 1.5 feet beyond the upper landing, or edge of the roof, thus exposing employees to fall hazards.   The Jonathan Ayala was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR  1926.1053(b)(1), which was contained in OSHA inspection number 1704177, citation number 1, item number 2 and was affirmed as a final order on 1/3/2025, with respect to a workplace located at 1233 Grand Canyon Drive in Wentzville, MO.

An employee was exposed to fall hazards when he faced away from the ladder while descending from a roof over 10 feet high.

The employees were not trained in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury while at the job site located at 5346 Burgundy Ct. in Imperial, MO. The employees were conducting roofing operations that exposed them to the hazard of falling from a height of greater than ten (10) feet.
