OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees that were exposed to steam burns and struck by hazards:   (a)  (a) #6 Furnace: On August 12, 2025, the employer used a backhoe to remove accumulated slag and debris from the shutdown furnace.  Residual water from an equipment cooling process was present in the backhoe bucket and had not fully evaporated. The wet bucket was placed into the furnace, resulting in a steam explosion.    Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this  abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photograph or video evidence of  abatement or other written records.

29 CFR  1910.132(a):  Employer did not provide proper protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact.  (a)  #6 Furnace:  On August 12, 2025,  an employee was exposed to high temperature steam while operating a backhoe to remove slag and debris from the furnace.  The employer did not ensure the employee was provided with adequate personal protective equipment such as heat resistant non permeable equipment to protect the face, head, torso and extremities.  Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement.
