On or about August 14, 2025, and at times prior, two employees installing siding were exposed to fall hazards greater than 10 feet while working on a pump jack scaffold. The employer failed to ensure employees were protected from falls by installing guardrail systems along all open sides, including the ends of the platform. The two sides were left open. The employer failed to ensure employees were protected from falls by having employees utilize a fall protection system or guardrails.  Abatement certification and documentation are required for this item (see enclosed "Certification of Corrective Action Worksheet").  Beltran's Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard (29 CFR 1926.501(b)(13)), which was contained in OSHA inspection number 1533698, citation number 1, item number 1 and was affirmed as a final order on July 14, 2021, with respect to a workplace located at 6887 Volga Dr. Colorado Springs, CO 80925.
