29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards: (a) 3769 E 135th Dr. Thornton CO, 80241: On or about July 2, 2025, employees were exposed to fall hazards of approximately 18 feet while conducting roofing activities on the top plate of the second level of an existing roof without utilizing personal fall protection or guardrails. The employer did not provide training on items such as, but not limited to; nature of fall hazards in the work area; correct procedures for inspecting the fall protection systems to be used; use and operation of guardrail systems, personal fall arrest systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used; limitations on the use of mechanical equipment during the performance of roofing work.
