29 CFR 1926.100(a):Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets. On or about August 15, 2025, during the installation of siding material at a job site located at 1040 Canyon View Rd., Sagamore Hills Ohio 44067; during the installation of roofing material, the employer did not ensure employees working on the ground were protected from possible danger of head injury from impact, or from falling or flying objects, falling debris from working surfaces of heights approximately 9.5 feet.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): On or about August 15, 2025, employees were exposed to a fall of approximately 9.5 feet while performing roofing activities on a multi family residential building located at 1040 Canyon View Rd., Sagamore Hills Ohio 44067 . The employees were working on all areas of the roof and the employer did not ensure that they were using fall protection. The Alfredo Construction LLC company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.501(b)(1), which was contained in OSHA inspection number 1836640, citation number 1 item number 1a and was affirmed as a final order on 8/14/2025, with respect to a workplace located at 1684 Sunnyside Ave., Akron Ohio 44303. The Alfredo Construction LLC company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.501(b)(1), which was contained in OSHA inspection number 1518401, citation number 1 item number 1a and was affirmed as a final order on 4/16/2021, with respect to a workplace located at 1000 East Turkeyfoot, Akron, Ohio 44312.
