29 CFR  1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  (a)P & M Steel Company at 4401 North Westport Avenue, Sioux Falls, SD 57107: On or about and at times prior to August 20, 2025, the employer did not ensure adequate guarding was in place on the vertical bandsaw, Marvel Series 8 Mark III, Model Number 8-Mark-III, Serial Number 840168.  This condition exposed employees to an amputation hazard.  The P & M Steel Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(1), which was contained in OSHA inspection number 1558638, citation number 1, item number 4 and was affirmed as a final order on 2/14/2022, with respect to a workplace located at 4401 North Westport Avenue, Sioux Falls SD 57107.

29 CFR  1910.219(c)(2)(i):All exposed part(s) of horizontal shafting seven (7) feet or less from floor or working platform were not protected by stationary casing(s) enclosing shafting completely or by trough(s) enclosing sides and top or sides and bottom of shafting:  (a)P & M Steel Company at 4401 North Westport Avenue, Sioux Falls, SD 57107: On or about and at times prior to August 20, 2025, the employer did not ensure that the cover was attached to the backside on the Marvel Series 8 Mark III, Serial Number 840168, as a rotating shaft was powering the vertical bandsaw.  This condition exposed employee's to caught in hazards with the rotating shaft.

29 CFR  1910.134(e)(1): The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace:   (a)P & M Steel Company at 4401 North Westport Avenue, Sioux Falls, SD 57107:  On or about and at times prior to August 20, 2025, the employer did not ensure that employees required to wear respirators were protected from respiratory illness or injury in that a medical evaluation was not performed prior to wearing required supplied tight-fitting respirators. This condition exposed employees to health hazards due to not being medically evaluated prior to respirator use.  P & M Steel Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.134(e)(1), which was contained in OSHA inspection number 1560801, citation number 1 item number 2b and was affirmed as a final order on 02/16/2022, with respect to a workplace located at 4401 North Westport Avenue,  Sioux Falls, SD 57107.

29 CFR  1910.305(g)(1)(iv)(A):Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure:  (a)P & M Steel Company at 4401 North Westport Avenue, Sioux Falls, SD 57107: On or about and at times prior to August 20, 2025, the employer did not ensure that an extension cord was not being used as a substitue for fixed wiring in that it was being used to power the SL Waber power strip / computer within Bay 3. This condition exposed employees to electrical hazards.
