29 CFR  1926.501(b)(2)(i): Each employee who was constructing a leading edge 6 feet or more above lower levels was not protected from falling by guardrail systems, safety net systems, or personal fall arrest systems:   a) Jobsite - 106 Blackford Blvd, Lebanon, PA: On or about August 27, 2025, employees performing leading-edge work while installing plywood decking on a flat roof of a four-story hotel without the benefit of a fall protection system, exposing employees to a fall of approximately 10 feet to the floor below.   b) Jobsite - 106 Blackford Blvd, Lebanon, PA: On or about August 27, 2025, employees performing leading-edge work while installing plywood decking on a flat roof of a four-story hotel without the benefit of a fall protection system, exposing employees to a fall of approximately 42 feet to the concrete below.   Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
