29 CFR 1910.176(b): Material stored in tiers was not stacked, blocked, interlocked or limited in height so that it was stable and secure against sliding and collapse: (a) Facility: On or about August 26, 2025, a stack of approximately 9 improperly stacked pallets was manually pushed from position 1 to position 2 at location 23-280-11 (4th level high in the racking system). Once they reach the end of position 2 the bottom pallet hits the end stops (1 on each side). The top approximate 6-7 pallets teetered for a second or two before falling into aisle 23 and striking an employee who was operating a tugger in aisle 23.
