29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  a) Jobsite located at 606 E. Baltimore Pike, Media, PA 19063: The employer did not ensure that employees engaged in roofing activities 6 feet or more above lower levels were protected by a guardrail system, safety net system, or personal fall arrest system. Employees were performing roofing activities on top of a three-story building without the use of fall protection,  thereby exposing employees to fall hazards approximately to 35 feet.  Conditions noted on or about August 25, 2025

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access:  a) Jobsite located at 606 E. Baltimore Pike, Media, PA 19063.: The employer did not ensure that the ladder used to access the upper landing surface of the roof was extended to at least 3 feet.  Employees used a ladder that was not extended at least 3 feet to access the upper landing surface of the roof to gain access to the roof, thereby exposing employees to fall hazards approximately to 35 feet.  Conditions noted on or about August 25, 2025

29 CFR  1926.20(b)(1):The employer did not create or maintain a safety and health program to include, but not limited to, identification and control of hazards, training and education, and work safety rules and procedures to enable employees exposed to fall hazards to recognize and avoid unsafe conditions at the workplace:  a) Jobsite located at 606 E. Baltimore Pike, Media, PA 19063: The employer did not initiate or maintain an accident prevention program to address the associated hazards that employees are exposed to during roofing activities, such as but not limited to fall hazards.   Conditions Noted on or about August 25, 2025.  TGBG Contracting was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.20(b)(1), which was contained in OSHA inspection number 1709573, citation number 1, item number 1 and was affirmed as a final order on 6/7/2024, with respect to a workplace located at 2438 Bryn Mawr Ave. Ardmore PA 19131.

Baltimore Pike, Media, PA 19063: The employer did not provide effective training for each employee conducting roofing activities from the roof of a three-story commercial building, and exposed to fall hazards of approximately 35 feet, in the recognition and avoidance of fall hazards.  Conditions Noted on or about August 25, 2025.  TGBG Contracting was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.503(a)(1), which was contained in OSHA inspection number 1709573, citation number 1, item number 2 and was affirmed as a final order on 6/7/2024, with respect to a workplace located at 2438 Bryn Mawr Ave. Ardmore PA 19131.
