29 CFR  1926.20(b)(1):The employer did not initiate and maintain such programs as may be necessary to comply with this part:  a) Jobsite located at 1445 Kyneton Road, Villanova, PA. - The employer did not develop and implement an accident prevention program to include, but not limited to, identification and control of hazards, training and education, and work safety rules and procedures to enable employees to recognize and avoid unsafe conditions and hazards.  Condition was noted on or about September 2, 2025.

29 CFR 1926.451(b)(1):Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports  as specified in paragraphs 1926.451(b)(1)(i)-(ii):  a) Jobsite located at 1445 Kyneton Road, Villanova, PA. -The employer did not ensure that the scaffold was fully planked. Employees were engaged masonry activities while working from different levels on a scaffold; thus, exposing the employee to fall hazards of up to approximately 13 feet.  Condition was observed on or about September 2, 2025.

29 CFR  1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Scaffold frame was used as a means of access:  a) Jobsite located at 1445 Kyneton Road, Villanova, PA. - The employer did not ensure that an employee was provided a safe means to access and egress the scaffold. Employee had to access and egress the scaffold by climbing on the scaffold frames and crossbraces; thus exposing employee to fall hazards.  Condition observed on or about September 2, 2025.

Condition observed on or about September 2, 2025.
