(a) On September 2, 2025 and times prior, at 1812 Gateway Drive, Grand Forks, ND, employees were not protected from caught in or pinching/ crushing hazards when operating a Tennsmith SR48P roller while rolling metal when there were no methods in place to keep body parts from the point of operation.
