29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: (a) On or about September 4, 2025, employees working on the ground as a part of a roofing crew to assist the roofers who were working directly above, were not protected from struck by hazards at heights of 20 feet or greater of falling materials and tools by wearing a protective helmet.

This exposed employees to a fall hazard of in-excess of 30-feet onto concrete.
