(a) At 15001 Gasparilla Road Placida, Florida 33946: On or about 9/5/2025, employees unloading or loading vessels from forks of a forklift were exposed to fall, struck by and drowning hazards when the dock employees were stepping off of the edge of the dock to get into the vessel.
