Hazards associated to the roof work were fire hazards using torches, combustible materials, burns to employees, falls, using portable saws and other power tools. This was documented on or about 09/10/2025.

29 CFR  1926.102(a)(1):The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  At a commercial worksite located at 1227 E Martha Truman Rd, Grandview MO -- employees engaged in roofing duties using portable saws, hammers, and other various hand tools were not wearing any eye protection. This exposed employees to getting shrapnel and other debris in their eyes causing injury. This was documented on or about 09/10/2025.

29 CFR  1926.501(b)(4)(i):Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes.  At a commercial worksite located at 1227 E Martha Truman Rd, Grandview MO -- employees were engaged in repairing and installing a new flat roof. An employee carrying plywood with a co-worker fell through a hole in the apron of the roof as they were placing the plywood in position. The employee was not wearing or using an personal fall protection resulting in him falling approximately 20' to ground level. This was documented on or about 09/10/2025.

At a commercial worksite located at 1227 E Martha Truman Rd, Grandview MO -- On 08/26/2025, an employee fell through a roof, and was taken via ambulance to the hospital. The employee was admitted for treatment of injuries. The employer did not report the hospitalization to the OSHA within 24 hours.
