29 CFR  1910.134(c)(1):A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:  (a) Rose Mill Company, LLC, 100 Brook Street, West Hartford, CT 06110: On or about September 10, 2025, and at times prior, employees were required to wear Moldex 2200N95 series respirators to conduct bagging, pulverizing, and mixing activities of both hazardous and non-hazardous substances. The employer has not established and implemented a respiratory protection program for employees required to wear respirators.

29 CFR  1910.134(d)(1)(i): Selection of appropriate respirators was not based on the respiratory hazard(s) to which the worker was exposed and user factors that affect respirator performance and reliability:   (a) Rose Mill Company, LLC, 100 Brook Street, West Hartford, CT 06110: On or about September 10, 2025, and at times prior, employees were using N95 respirators to conduct daily job duties when the employer had not conducted an assessment of the respiratory hazards in the production area. The employer provided N95 respirators without conducting a risk assessment to determine if the facility had appropriate ventilation to control exposure to anhydrous borax.

29 CFR  1910.1200(e)(1):Employer had not developed or implemented a written hazard communication program that included the requirements outlined in 29 CFR 1910.1200(e)(1)(i) and (e)(1)(ii):   (a) Rose Mill Company, LLC, 100 Brook Street, West Hartford, CT 06110: On or about September 10, 2025, and at times prior, the employer had not implemented a written hazard communication program to address the hazards of chemicals that employees were exposed to including, but not limited to: anhydrous borax, borax 5 mol, borax 10 mol, boric acid boric oxide, disodium octaborate tetrahydrate, soda ash,  Exposure to these chemicals can cause skin irritation, eye irritation, and flammable hazards.
