29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c). (a) On or about September 10, 2025 at the jobsite located at the intersection of Capitol Drive and Sussex Street, Pewaukee WI employees were performing work in an approximately six foot and ten inch deep excavation without adequate cave-in protection, thereby exposing employees to a struck-by / crushing hazard. American Sewer Services, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.652(a)(1), which was contained in OSHA inspection number 1621595, citation number 1, item number 2 and was affirmed as a final order on February 7, 2023, with respect to a workplace located at 8001 N Port Washington Rd., Fox Point, WI .
