On or about September 10, 2025, at an establishment located at 825 East Wisconsin Ave, Appleton WI, employees were required to perform servicing and maintenance tasks on Coater 17 fand the procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques needed to follow normal stopping procedure, the type and location of machine operating controls, how to deactivate the energy sources, and how to specifically verify that the machine has been deenergized.
