29 CFR 1910.37(b)(6): Each exit sign must be illuminated to a surface value of at least five foot-candles (54 lux) by a reliable light source and be distinctive in color. Self-luminous or electroluminescent signs that have a minimum luminance surface value of at least .06 footlamberts (0.21 cd/m2) are permitted. On or about September 10, 2025, in Building 17 at a facility located at 825 E Wisconsin Avenue in Appleton, Wisconsin, employees were working in the loading dock area without Door 102 exit sign illuminated.

29 CFR 1910.147(c)(4)(ii):The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following: (A) A specific statement of the intended use of the procedure; (B) Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy; (C) Specific procedural steps for the placement, removal and transfer of lockout devices or tagout devices and the responsibility for them; and (D) Specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures. On or about September 10, 2025, at an establishment located at 825 East Wisconsin Ave, Appleton WI, employees were required to perform servicing and maintenance tasks on Coater 17 fand the procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques needed to follow normal stopping procedure, the type and location of machine operating controls, how to deactivate the energy sources, and how to specifically verify that the machine has been deenergized.
