29 CFR 1910.132(d)(1)(i): When the employer had assessed the workplace hazard(s) and determined that hazard(s) were present, the employer did not select and/or use the types of personal protective equipment that would protect the affected employee from the hazards that had been identified. a) Fluorescent Penetrating Inspection Line (FPI) - Spray Booth and Wash Station: On or about September 11, 2025, the employer did not select or provide appropriate personal protective equipment, such as goggles and face shields, necessary to adequately protect employees from splash hazards during spraying and rinsing operations in the Spray Booth and at the Wash Station. Employees working along the penetrant line were exposed to splash risks from corrosive chemicals including Method A: Water-Washable emulsifier penetrants containing a mixture of ethoxylated alcohols (ZL-67 and ZL-37) classified as a Category 1 eye-damaging agent, as well as other chemicals. b) Fluorescent Penetrating Inspection Line (FPI) - UV Light Booth: On or about September 11, 2025, the employer did not select or utilize personal protective equipment designed to safeguard employees from ultraviolet A (UV-A) rays generated by high-intensity ultraviolet radiation within the wavelength range of 320-400 nanometers. During fluorescent liquid penetrant inspection operations, employees were exposed without the benefit of UV-blocking eyewear and polycarbonate face shields necessary to shield the eyes and face from potentially hazardous UV-A rays emitted by both overhead LED UV-A lighting and hand-held UV-A lights in the inspection area.

29 CFR 1910.133(a)(1):The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation: Fluorescent Penetrating Inspection Line (FPI): a) On or about September 11, 2025, employees working along the penetrant line were exposed to splash hazards from corrosive liquids including ethoxylated alcohols (ZL-67, Category 1 eye damaging agent) and acetone (Acetone, Category 2 eye irritating agent), among others. The employer did not ensure that inspectors were provided the appropriate eye or face protection when exposed to eye or face hazards from liquid chemicals in the following locations: 1. Spray-on Penetrant Booth 2. Dipping Penetrant Booth b) On or about September 11, 2025, employees working along the penetrant line were exposed to ultraviolet radiation within the 320 to 400 nanometer spectral range without adequate protective equipment. The employer did not implement adequate protective measures to safeguard employees' skin or eyes while working in the following hazardous location along the FPI line: 1. UV-light Booth
