29 CFR 1910.132(d)(1)(i): The employer did not ensure that each affected employee was properly trained and used the Personal Protective Equipment (PPE) that will protect the affected employee from the hazards identified in the hazard assessment: a)Autozone Parts, Inc. at 414 East North Street, Rapid City, SD 57701: On or about and at times prior to September 12, 2025, the employer did not ensure that employees were utilizing proper PPE while conducting installation and charging of vehicle batteries as well as clean up of spills of vehicle battery acid. This condition exposed employees to chemical burns and eye damage.

This condition exposed employees to chemical burns and eye damage.

29 CFR 1910.1200(f)(6): The employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the information specified under paragraphs (f)(6)(i) through (ii) of this section: a)Autozone Parts, Inc. at 414 East North Street, Rapid City, SD 57701: On or about and at times prior to September 12, 2025, the employer did not ensure that all hazardous chemicals in the workplace were properly labeled. Sodium Carbonate (Soda Ash) had been removed from its original container and placed in an unlabeled black bucket. Lack of effective labeling exposed employees to injuries with improper use of chemicals and emergency procedures.
