Employees were exposed to serious struck by injuries in that the employer didn't require daily inspection of a York fork truck that was in use. Employees operated the York fork truck with the rubber pad to the taped to the gas pedal.

Employees in the warehouse were exposed to serious electrical shock, burn and or electrocution injuries, in that the electrical equipment operating at 50 volts or more was not guarded or enclosed to protect from accidental contact, including but not limited to the storage area on the eastside of the warehouse, in that employer did not have a guard that fully enclosed the electrical connections.
