29 CFR 1926.1053(b)(1):Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: On or about September 16th, 2025, employees engaged in roofing operations located at 1221 West 6th Street, Papillion, Nebraska 68046 were exposed to fall hazards in that employees were using an extension ladder for access and egress to and from the roof. The ladder did not extend at least 3 feet above the upper landing surface.

29 CFR 1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system. The employer is failing to protect employees from fall hazards. On or about September 16th, 2025, employees were engaged in roofing operations located at 1221 West 6th Street, Papillion, Nebraska 68046, were exposed to fall hazards in that employees were observed working at heights greater that 30 feet without being connected to their restraining devices. CJ Fanning Construction Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1757967, citation number 1, item number 1 and was affirmed as a final order on 7-9-24, with respect to a workplace located at 6232 North 155th Ave, Omaha, Nebraska 68116. CJ Fanning Construction Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1641990, citation number 1, item number 2 and was affirmed as a final order on 3-8-23, with respect to a workplace located at 1512 Bristol Street, Papillion, Nebraska 68046.
