29 CFR  1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolate:   (a) Production- On September 4, 2025, the employer did not ensure employees servicing equipment were trained as "authorized" employees. Employees assigned the task of unjamming the shear on Control Line 1 were not provided with the knowledge and skills needed for the safe application, usage, and removal of energy control.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR  1910.147(d):The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):  (a) Production : On September 4, 2025, employees were exposed to machine hazards associated with moving parts when unjamming at/near the shear on equipment such as but not limited to Control Line 1. The employer did not implement energy control application steps such as but not limited to lockout tagout when performing these tasks [per the 1910.147(d)(2) requirements]. As a result, the remaining applicable energy control elements, involving dissipation machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine servicing hazards.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. 29 CFR  1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):   (a) Production - On September 16, 2025,  the point of operation of the shear on Control Line 1 was not guarded exposing employees to amputation hazards during operation.   In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.  Voestalpine Precision Strip, LLC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(3)(ii) which was contained in OSHA Inspection Number 1748576, Citation 1, Item 1a, and was affirmed final order on November 12, 2024, with respect to the workplace located at 7888 102nd Street, Pleasant Prairie, Wisconsin, 53158.

29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours, either by telephone or in person to the OSHA Area Office that is nearest to the site of incident, by telephone to the OSHA toll-free central telephone number, 1-800-321-OSHA (1-800-321-6742), or by electronic submission using the reporting application located on the OSHA public web site at www.osha.gov:   (a) The employer did not report an in-patient hospitalization that was the result of a work-related incident that occurred on or about September 4, 2025, to OSHA within twenty-four (24) hours. The injury was reported to OSHA on September 16, 2024.  No abatement certification or documentation is required for this item.

29 CFR  1910.1200(f)(6)(ii): Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals and which, in conjunction with the other information immediately available to employees under the hazard communication program, would provide employees with the specific information regarding the physical and health hazards of the hazardous chemical:  (a) Production: On September 17, 2025, the employer did not ensure that each secondary container containing hazardous material were labeled. Employees were exposed to chemicals, including but not limited to Videojet Cleaning Solution V901-Q chemical.  No abatement certification or documentation is required for this item.
