29 CFR  1910.334(a)(2)(i):Portable cord and plug connected equipment and flexible cord sets (extension cords) shall be visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket). Cord and plug connected equipment and flexible cord sets (extension cords) which remain connected once they are put in place and are not exposed to damage need not be visually inspected until they are relocated.  a)On or about September 17, 2025, the employer failed to ensure that the flexible cord set used for the mobile scissor lift table in the "set up area" was visually inspected for external defects before use. The cord was wrapped in multiple locations with electrical tape, which prevented the employee using the equipment from visually inspecting the outer jacket and insulation for damage. This failure exposed the employee to potential electrical shock and fire hazards from using a cord with unassessed damage.

29 CFR  1910.305(a)(2)(x):Flexible cords and cables shall be protected from accidental damage, as might be caused, for example, by sharp corners, projections, and doorways or other pinch points.  a) On or about September 17, 2025, in the "set up area," the employer failed to protect a flexible electrical cord from accidental damage. The cord, which powered a mobile scissor lift table, was run across the floor where it was subjected to being run over by the lift and snagged on other objects. As a result, employees were exposed to electrical shock and fire hazards from damaged cords.

29 CFR  1910.305(g)(1)(iv):Unless specifically permitted otherwise in paragraph (g)(1)(ii) of this section, flexible cords and cables may not be used:  1910.305(g)(1)(iv)(A) As a substitute for the fixed wiring of a structure;  a) On or about September 17, 2025, in the "set up area," the employer improperly used a flexible cord as a substitute for the fixed wiring of the structure. A flexible extension cord was secured by metal clips and tape over the plug while being used as a permanent, ongoing power supply for a mobile scissor lift that operated daily along a fixed path. As a result, employees were exposed to electrical shock and fire hazards from a cord being used in a manner for which it was not designed, leading to excessive wear and damage.

29 CFR  1910.22(a)(1):All places of employment, passageways, storerooms, service rooms, and walking-working surfaces are kept in a clean, orderly, and sanitary condition.  a) On or about September 17, 2025, the employer failed to keep a passageway in the "set up area" in an orderly condition, in that a flexible electrical cord for a mobile scissor lift table (Manufacturer: MarMax Automation, Inc.) was laying  uncovered on the ground in the set up area aisle way where employee had to step over to enter and exit the area. As a result, employees were exposed to slip, trip and fall hazards.  The Custom Cast Marbleworks, Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1453424, citation number 1, item number 1 and was affirmed as a final order on 01-21-2022, with respect to a workplace located at 3154 Exon Avenue CINCINNATI, OH 45241 UNITED STATES OF AMERICA.
