29 CFR  1926.502(d)(15): Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached nor was it designed, installed and used as follows:  As part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person.  a.  On or about September 17, 2025, at the above-mentioned jobsite, employees conducting roofing operations on a 12:12 pitch roof, approximately ten (10) feet nine (9) inches above the ground, were connected to the same anchorage point, thus being exposed to fall hazards.

On or about September 17, 2025, at the above-mentioned jobsite, employees were exposed to fall hazards of approximately ten (10) feet nine (9) inches above the ground while conducting roofing operations on a residential property.  Two employees were observed not using fall protection correctly when the employees were tied to a rope grab connected to their waist D-Rings.

On or about September 17, 2025, at the above-mentioned jobsite, the employer exposed employees to fall hazards, in that, employees were using separated components of an aluminum extension ladder to access roof of garage during roofing operations, and these separated ladder components did not have cleats/feet.
